Do not enter or send sensitive personal or health-related details through this website, by text message, or by email. Medical information is gathered only during care, through appropriate channels.

What we collect

If you join the membership waitlist, we collect the details you enter: first and last name, phone number, email address, local address, and whether the membership is for an individual or a family. We also record the date and time you submitted this form.

If you text or email us to ask about a visit, we receive your phone number or email address and whatever you choose to write.

How we use it

We use this information only to contact you about your interest in receiving medical concierge care, such as scheduling questions, answering your inquiry, and letting you know when membership becomes available. We do not sell your information and we do not share it for advertising.

Where it is stored

Waitlist details are stored in a secured database with our website hosting provider, Cloudflare, and are viewable only by Zahav MD staff. Like most websites, our host may keep standard technical logs, such as IP address and browser type, for security and reliability.

Cookies and tracking

This website does not use advertising cookies or analytics trackers.

Removing your information

You can ask us at any time to remove you from the waitlist or delete the details you gave us. Email aaron@zahavmd.com.

Patient records

This notice covers this website only. Information we create or receive while providing medical care is handled under separate rules for medical records and will be explained to patients directly.

We may update this notice from time to time. The date at the top shows when it was last changed.
